All eyes and ears are on the Chief at the start of this episode. He's about to name who will get to do the solo surgery. We already know it's going to be Cristina, but the catch is that she doesn't get to do the surgery and instead has to decide who should get to do it in her place. Remember this is her punishment for sort of although not really knowing about last ep's intern sacrifice experiment.
Cristina's punishment doesn't end there though, she's also gets to spend the day with Dr. Dixon who is back for another guest appearance, and how lucky we are. The offense to those with Asperger's Syndrome, but I really don't think Dixon is cut out to be a doctor given she can't connect on any level with patients. Meanwhile, Lexie will be working with McSteamy and there is definitely some sexual tension there. She's still on probation though, and will be for quite some time, therefore he puts her on cappucino duty. Sadie is working with Alex and seems to have a thing for him because she asks Meredith to give her the scoop.
Sloane's McSteamy and Lexie's patient for the day is a woman who can't speak and hasn't been able to speak for about five years. She and her husband have been speaking, or not, through dirty looks and post-it notes. The wife basically ignores her husband and the husband is so fed up he's about at his wits end.
A major case falls in Alex and Sadie's lap while they're working in the ER. Two sisters are brought in after a bad car accident. The one is 16 and the other is 15 years old. The 16 year old was driving and texting at the same time when she crashed. Before they crashed the 15 year old offered to do the texting for her sister but she wanted to do it herself and then the accident happened. So now the 15 year old blames her sister and they fight the entire time they're in the ER. Alex tells them to shut up which they do momentarily but then the fighting continues, well until the 16 year old starts having major complications and they have to wheel her away. As they're wheeling her away the 15 year old is still yelling at her sister and the last words she yells at her before she's wheeled out of the room is "I hope you die." I think we all know this one isn't going to have a happy ending, sorry!
With Cristina holding the reigns over who gets to do the solo surgery most of the episode everyone is trying to suck upto her so she will choose them. George is the first to start sucking up. He tells her that sure he had trouble in the beginning but he's come out the other side better and smarter, he never gave up because he's a fighter. Meredith is the next to talk to Cristina and she obviously thinks because she and Cristina are BFF's that she has this in the bag, but she has another thing coming.
Callie has to do a bone scan for one of the sisters in the car accident and as she's looking over the scans Sadie comes by and starts asking questions. Callie gets all flustered and has to leave the room. Callie is clearly a little sexually on edge since Hahn left. I think she needs to pay McSteamy a 2 minute visit to take care of that. She does go to McSteamy but instead she's just looking for some advice, he doesn't really care to hear about it much, but she gets all excited at the thought of Sadie smiling at her. Since when did Callie turn so lesbian? I mean, I get that she is a lesbian but it wasn't that long ago she was straight and married to George. Now she's acting like a kid in a candy shop over a girl smiling at her.
The 16 year old girl ends up needing surgery. In surgery Derek lets Alex step in, which Alex is both excited and slightly nervous about, and as soon as Alex starts there are complications. As I mentioned before the story doesn't end well. The girl ends up being brain dead. And, all that Dixon has to say about this is that the girl will be a perfect candidate as an organ donor given that she's so young. I'm not sure if I like or hate Dixon being around. In this instance it's almost nice to have her around because she lightens the scene a little with her stupid comments but then it gets annoying. Do you like her or not?
Izzie is covering the clinic today, or at least she's supposed to be, but then she decides she wants to go off to another room to have sex with dead Denny. Yes, unfortunately, dead Denny is still lingering around. I'm trying not to mention him much hoping that if I ignore him he'll go away, I wish Izzie would just do the same. So anyway, George is pissed that he has to cover the clinic and goes to Alex to vent. He says he thinks there's something wrong with Izzie, Alex stops him there and tells him to never talk to him about Izzie again.
Alex goes to Cristina, in the bathroom no less, and he tries his best to be a suck up but he knows he's no good at it so he gives he a straightforward talk. He says that he's the most qualified for the surgery and he should be the one that gets it. Short and to the point is his strategy.
Sadie and Lexie observe the surgery on the woman who can't speak. As they're observing from above Sadie complains to Lexie how broken the entire intern program is. Part of the problem is that everyone is sleeping together, and of course this turns to Sadie informing Lexie that McSteamy has the hots for her. Lexie acts all surprised but then she smiles. You know she's thinking she wants to order a McSteamy with a large fry. During the surgery, McSteamy lets Bailey in on some of the action and Bailey acts like one of the residents, she's so excited to get to learn something new. It's good to see Bailey get so excited over something. I'm tired of the grumpy, bossy Bailey. It's time for Bailey to let loose and get funky with her bad self.
Dixon has to tell the sister and family of the 16 year old that she is brain dead. The sister takes it the worst, screaming and crying and trying to shake her sister awake. The whole time Dixon just keeps going on and on about how they need to make a decision about her being an organ donor because there are people dying that could use those organs. She doesn't even given the family a moment to grieve before she starts clawing for the organs, she's like an animal. Seriously have some R-E-S-P-E-C-T.
Izzie goes to Cristina, but not to be a kiss ass, instead she just wants to tell Cristina all about the amazing sex she's having. She leaves out the part how the sex is with a DEAD guy! Izzie must have a brain tumor... you don't just go off the deep end overnight without something being wrong in your head.
The woman who couldn't talk before, still isn't talking after surgery. Her husband can't take it anymore and starts yelling at her. Lexie calmly suggests the guy go for a walk or a drive to settle down. After he and McSteamy leave the room Lexie has a little chat with the woman and encourages her to try just saying hi. The woman eventually does and is able to work out a whisper at first and then after a few minutes is able to work upto somewhat of a normal speaking voice. Her husband comes back a little while later and she says hi to him and they both breakdown crying.
Cristina talks to Dixon who is still mad that the family of the dead 16 year old have yet to make a decision. She tells Cristina that they're just letting emotions get in the way of a very important decision. This seems to snap some sense into Cristina and she's finally able to make a decision, which she had been painfully trying to make all day long. She lines up the residents and then goes to the OR board and writes... "Karev" on the board. She has chosen Alex to do the solo surgery. Earlier in the day he and Izzie had promised each other than if either were chosen they would choose the other to assist them. So, Alex goes around the hospital trying to find Izzie before finally finding her in bed, with dead Denny, though sane Alex can't see him. He tells Izzie the good news and then tells her that he loves her. This is the first time he has spoken those words so it's a major thing. He says he realizes why she's been so distant lately, it's because she loves him too. Umm.. no you dummy, don't you get it.. she's sleeping with the ghost of her dead fiance.
After giving his heart to Izzie Alex tries to prepare himself for the solo surgery but he's freaking now. His hands are shaky and now he thinks Izzie won't show up to assist him in the surgery because of what he just told her, so he pages Meredith as a back-up. Meredith is up on the observation deck getting ready to watch the solo surgery with everyone else. She and Cristina are in the middle a bitter battle over why Cristina did not choose her, whether it was about their personal fight or just that Cristina felt Alex was better qualified. Cristina tells Meredith that unlike her she knows how to separate her personal life from work and that's why chose Alex. Hmm, I know it's been a while but doesn't Cristina remember some guy named Preston Burke, you know she kind of met him at work and almost married him. I don't mean to bad mouth Cristina here though because I do actually agree with her on this one. Cristina ends up getting mad and stands up and tells everyone in the observation room that they are all just as at fault as she is about the intern experimentation, it wasn't just her interns involved. She then storms out of the room. George quickly takes Cristina's seat next to Meredith and he instantly starts telling her that he thinks something is wrong with Izzie, then Meredith gets the page from Alex and leaves. McMacho, aka Hunt, runs after Cristina. When he catches up with her he leads her downstairs to the furnace room. There are vents on the floor so he leads her over the vents and air blows through their hair and they kiss. It makes for a perfect cheesy makeout scene.
Meredith goes to Alex after his page and he tells her what he told Izzie and how he's afraid she won't show up. But just a moment later Izzie does show up. Meredith then goes to the 15 year old sister who still can't quit crying. She's obviously grieving her sister's death but she also feels horrible that the last words she told her sister were "I hope you die." Meredith tells her that no matter what she said her sister loved her and that's all that matters.
After Meredith calms the sister down she and Derek talk, he seems really depressed. He tells her that he can't take any more death. That makes at least two of us.
Lexie goes to McSteamy's after work and they make some McNasty.
The episode ends with Izzie and Alex getting ready to do the below-the-knee amputation solo surgery. Alex is just getting ready to start cutting into the leg as it ends.
